厚生労働省は2026年6月12日、2023年1月4日から2025年10月29日までに省内で送信された『Microsoft Teams』のチャット記録が消失したと発表した。期間は約2年10か月分にわたり、原因はシステム運用を委託していた東芝の作業ミス。消えたデータのうち行政文書に該当する一部チャットは復元が困難だという。森友問題以降、行政文書の電子保存が進められていたなかでの大規模消失で、国会答弁の根拠資料が失われる可能性もあり、野党や市民からは厳しい批判の声が上がっている。
